Our Care Teams

Our Care Leaders and Care Partners listen to you. You are what matters most to us.

Our Care Leaders are registered nurses or allied health professionals who will visit you to discuss care that keeps you in control of your own life providing you with information that is simple and easy to understand. Your unique story will lead the suggestions we make and the support we offer. Our Care Leaders will focus on what you really want and what you really need to stay living safely and confidently at home. They will also help your family to find the right information and the support they need.

Our Care Partners are expected to have a Certificate in Home and Community Care or Aged Care; some have other care qualifications and most have completed dementia care courses. Our Care Partners are a part of a small group of staff who get to know you well. Most days you will already know the person who is coming to see you to provide you with support. Your Care Partner may help you to do your shopping or get to appointments, take you on short outings or to visit friends.
